However, over the past few weeks, the client seems to be diverting attention away from himself and toward the nurse. A nurse has gone through the preparation and orientation phases of a therapeutic relationship with a client and has moved into the working phase. Now, the client does not seem to understand the nurse’s role. At times, the client wants the nurse to make decisions for her, but at other times, she is unreceptive to the nurse’s therapeutic suggestions. In general, the client’s listening skills are poor. How should the nurse intervene? 3. A client with paranoid schizophrenia has made a great deal of improvement. His hallucinations have abated; he is taking his medications, is compliant, and is nonviolent. As the nurse is planning his discharge and discussing it with him and other members of the health care team, the client states, “I am not ready to be discharged yet.
