To understand the purpose of the Clinical Documentation Improvement programs, it is first important to know what Clinical Documentation is. Clinical Documentation is the main factor in an encounter with a patient and this has to be timely, accurate, and also reflect the scope of the services provided ("Clinical documentation improvement", n.d.). Thus, the purpose of the Clinical Documentation Improvement program is to make the accurate representation of a patient’s clinical status easier and smooth and then translate that status into a coded data. After this translation it is again translated into reimbursement, quality reporting, public health data, physician report cards, and disease tracking and trending ("Clinical documentation improvement", n.d.).
